Metabolic health is the foundation of how we feel and function. It impacts our energy, weight, mood, hormones, and long-term risk for chronic diseases.

Yet the early warning signs of poor metabolic health often go unnoticed until one day, they don’t. That’s where biomarkers come in.

Among the many metrics that doctors can test, three in particular stand out as especially insightful when it comes to metabolic function:

  • C-Reactive Protein (CRP),
  • the Triglyceride-to-HDL Ratio (TG/HDL), and
  • HOMA-IR, a calculation that estimates insulin resistance.

These markers are simple to test, often inexpensive, and astonishingly revealing.

Let’s explore what each of these biomarkers tells us, why they matter, and how you can use them to build a healthier, more informed life.

CRP: The Inflammation Signal You Don’t Want to Ignore

C-reactive protein, or CRP, is one of the most reliable indicators of inflammation in the body. Produced by the liver, CRP levels rise when your body senses injury or infection.

That makes it a great tool in acute medical situations, but its value goes far beyond that. When CRP remains elevated for long periods, it suggests that the body is fighting a low-grade, chronic inflammatory state.

This kind of hidden inflammation is at the root of many chronic illnesses like heart disease, diabetes, autoimmune disorders, and even depression.

And it’s not always obvious. You may feel fine, but a persistently high CRP can reveal that your immune system is on high alert, often due to poor diet, stress, poor sleep, environmental toxins, or visceral fat (the fat that wraps around your organs).

What makes CRP particularly useful is its generality. While it doesn’t tell you exactly where the inflammation is or what’s causing it, it does give a clear signal that something isn’t right.

It’s a little like a smoke detector: it may not tell you which room the fire is in, but it’ll let you know something’s burning.

What do the CRP values indicate?

  • Below 1.0 mg/L is normal
  • Low levels (<1.0 mg/L) indicate low cardiovascular risk.
  • Moderate levels are in the range of 1.0-3.0 mg/L.
  • Levels above 3.0 mg/L could indicate higher cardiovascular risk.

CRP levels may spike for other reasons, ie, an infection; therefore, one-time tests should always be interpreted in context.

The good news is that CRP levels are reactive. With lifestyle changes such as lowering processed food intake, increasing movement, optimizing sleep quality, and managing stress, there is often a notable decrease in CRP levels in just a few weeks.

It’s one of the only markers that allows you to see movement with your time and effort.

TG/HDL Ratio: A Sign of How Your Body Handles Fat and Sugar

While cholesterol numbers still dominate most annual check-ups, they don’t always tell the full story.

Total cholesterol or even LDL (“bad” cholesterol) can be misleading in isolation. That’s where the triglyceride-to-HDL ratio comes into play, a far more nuanced indicator of metabolic health.

Triglycerides are a type of fat that circulates in your bloodstream. They rise when you eat more calories than you burn, especially from sugar and refined carbohydrates.

HDL, on the other hand, is often dubbed the “good” cholesterol because it helps remove excess fats from the blood.

The ratio between these two gives a clear indication of how well your body is managing lipids and, more importantly, how insulin sensitive you are.

What does the TG/HDL ratio indicate?

A low TG/HDL ratio—ideally under 2.0 in U.S. units (mg/dL)—is considered a strong indicator of good metabolic health and insulin sensitivity.

Conversely, a higher ratio is often an early sign of insulin resistance, a condition where your cells stop responding to insulin effectively.

When this happens, the body compensates by producing more insulin, which can eventually lead to Type 2 diabetes and other health issues.

Let’s say your triglycerides are 200 and your HDL is 40. That gives you a TG/HDL ratio of 5.0—a potential red flag.

On the other hand, triglycerides of 100 and HDL of 50 would give you a ratio of 2.0, which is much more favorable.

Improving this ratio isn’t about crash dieting or eliminating fat. It’s about shifting the quality of your diet, cutting refined sugars and processed foods, eating more fiber, increasing omega-3s, and moving your body daily.

These changes help your body use fat more efficiently and reduce triglyceride buildup in the blood.

HOMA-IR: The Quiet Predictor of Insulin Resistance

While most people are familiar with blood sugar tests like fasting glucose or A1c, few know about HOMA-IR, even though it might be the most telling marker of all.

HOMA-IR stands for Homeostatic Model Assessment of Insulin Resistance, and it provides an estimate of how well your body is responding to insulin.

Insulin is the hormone responsible for shuttling glucose out of your blood and into your cells. When the system is working well, glucose and insulin levels remain stable.

But when the body becomes insulin-resistant, more and more insulin is needed to keep blood sugar in check.

Over time, this overproduction wears out the pancreas and leads to elevated glucose levels—and eventually, diabetes.

Why is HOMA-IR a better indicator?

By the time fasting glucose finally begins to rise, the insulin resistance process could have been slowly happening for years.

That is why HOMA-IR is so useful; it can detect early metabolic suffering before it escalates to full-blown disease.

  • A HOMA-IR value below 1.0 is considered excellent,
  • Between 1.0 and 2.0 is considered generally healthy.
  • Above 2.0 indicates a degree of insulin resistance
  • Above 3.0 indicates a compromised state.

Biomarkers as Tools, Not Labels

It’s easy to feel overwhelmed by lab values or to be fixated on certain numbers. Remember, biomarkers are not worth it. Rather, biomarkers are instruments that can help you navigate your health.

What is so good about CRP, TG/HDL Ratio, and HOMA-IR is that they are not only diagnostic, but they are also directional.

CRP will help you see inflammation, which is probably the foundation for so much metabolic health.

The TG/HDL ratio will give you a glimpse of how well your body processes sugars and fats and is ultimately indicative of cardiovascular and insulin health.

HOMA-IR gives you a direct look into how well your body deals with insulin. Its early warning signs can even show you before they become a more serious health risk.

Collectively, these three markers provide a surprisingly complete picture of your metabolic health.

Conclusion

You don’t have to completely change your life overnight to see improvements in your metabolic health. It starts with curiosity.

Get your labs done. You’ll see where you’re at. Then pick one thing to work on, maybe that’s going for a walk after meals, switching out a sugary treat and getting a high-fiber food, or simply going to bed 30 minutes earlier.
